After your appointment please stop at the check-out window before exiting.

Payment for all services must be made at the time of your visit unless you have Traditional Medicare Part B. Medicare patients click the link below.

We can electronically file a claim for most private insurers if you bring your insurance card. Your insurance company will reimburse you directly at the “out-of-network” rate or apply it to your annual deductible.

All services should be paid in full at this time.
